Transition By HR Program

Program Information

Program Vision:

To provide holistic, person-centered support that empowers vulnerable individuals—particularly those navigating homelessness or aging out of care—with the tools, skills, and resources needed to transition into stable housing, employment, and meaningful independence.

Target Population:

Chronically homeless individuals

  • Youth and young adults experiencing homelessness

  • Youth in transition (ages 18–24)

  • Individuals aging out of foster care 

  • And those at risk

Program Components

Basic Needs

Addressing immediate needs such as food, clothing, hygiene kits, and shelter
Transportation assistance through bus passes, ride shares, or community transportation
partnerships

Life Skills

Interactive groups on budgeting, banking, meal prep, self-care, and communication
Hands-on experiences with home maintenance, time management, and goal setting
ï‚· Tenancy education and rights to prepare for permanent housing

Education Access

GED or high school diploma completion assistance
Partnerships with alternative education programs, community colleges, and trade schools. Tutoring support and internet access.

Employment Assistance

Resume-building and mock interviews tailored to each client’s goals. On-site job readiness groups (dressing for success, teamwork, team building, workplace
conduct)
Referrals and connections to vocational training, certifications, apprenticeships, and
employers
